SMS messages may be used to collect or provide limited protected health information from the parent such as: patient name, appointment information, billing information, information that identifies the practice as a therapy practice, and any pertinent clinical information. How is the information in the SMS messages used? These messages are used by the Tri-County Therapy team to update patient data in our software. These messages are also used to communicate information regarding therapy appointments and/or send progress updates. These SMS/text messages may not be secure forms of communication. What information do we share? SMS opt-in or phone numbers for the purpose of SMS are not being shared to any third party. SMS messages are only shared to the direct office member or therapist who is working with the patient who is receiving services.
Voice Messages: Tri-County Therapy may leave voice messages to the phone number(s) I have provided them. I understand that these messages may contain limited protected health information such as: patient name, appointment information, billing information, information that identifies the practice as a therapy practice, and any pertinent clinical information. I understand that these voice messages may not be secure forms of communication, and that by consenting to these communication types, I am waiving my rights to secure communications.
